Collaborative Care Partners, LLC (CCP) and its Participant and Preferred Providers are committed to equitable health care. Health Equity is the attainment of the highest level of health for all people, where everyone has a fair and just opportunity to attain their optimal health regardless of race, ethnicity, disability, sexual orientation, gender identity, socioeconomic status, geography, preferred language, or other factors that affect access to care and health outcomes.

 

The first step toward achieving health equity is identifying health disparities in specific populations.  These become referred to as Underserved Communities which share a particular characteristic, as well as geographic communities. This can include those who have been systematically denied a full opportunity to participate in aspects of economic, social, and civic life, such as Black, Latino, and Indigenous and Native American persons, Asian Americans and Pacific Islanders and other persons of color; members of religious minorities; lesbian, gay, bisexual, transgender, and queer (LGBTQ+) persons; persons with disabilities; persons who live in rural areas; and persons otherwise adversely affected by persistent poverty or inequality.

 

Through data collection and analysis, CCP has identified care gaps and health disparities among its Underserved Communities. Health-related data includes social risk factors, experience of care, and comprehensive patient demographic data, which are all valuable data elements to ensuring Health Equity for all patients.
