Because your doctor is a part of Collaborative Care Partners, LLC, there are services that may be available to you at no extra cost:

• One number to call for your MetroHealth scheduling needs (216-957-3001)

• An assigned health care coordinator who can help you manage your health care, arrange your appointments, and answer any questions you may have

• Educational events and materials to help you stay as healthy as possible

• Telehealth options, where available, so you can get needed care without leaving home

• Home visit options to ensure you are getting required care after any hospital stay

We invite you to identify your primary provider/physician either online at Medicare.gov or by filling out a paper-based form, which you can request by calling 216-957-3001.
