PROVIDERS

Collaborative Care Partners (CCP) helps its participating providers improve the overall health of patients who qualify for and choose traditional Medicare Fee for Service (FFS) benefits. CCP is a Direct Contracting Entity (DCE) as part of Medicare’s Global and Professional Direct Contracting (GPDC) model. CCP includes doctors, hospitals and other health care providers who agree to work together to keep patients healthy and ensure they get the care they need, when they need it. It is designed to improve care coordination and delivery, and better manage the health needs of patients.  

CCP GOALS:

Transform risk-sharing arrangements in Medicare fee-for-service (FFS).
