PROVIDERS
Collaborative Care Partners, LLC (CCP) helps its participating and preferred providers improve the overall health of patients who qualify for and choose traditional Medicare Fee for Service (FFS) benefits. CCP serves as an ACO in Medicare’s ACO REACH model, which includes doctors, hospitals and other health care providers who agree to work together to keep patients healthy and ensure they get the care they need when they need it. It is designed to improve care coordination and delivery, and better manage the health needs of patients.
